Not sure about a train shaped cake, though Wegmans can print a jpg onto a sheet cake.
Try Sweet Life Bakery on 22nd Street in Point Breeze. I'm a big fan of her cupcakes, and every time I'm in there, I've seen the owner working on some amazing children's birthday cakes. I'd be willing to bet she'd work with you to make what you have in mind.

I second that. The pastries at Night Kitchen are amazing. Speaking of Chestnut Hill, Bredenbeck's Bakery is also great place to try. They even have a picture of a "Thomas the Train" cake that they made on their website under the Cakes for Kids tab.
